Health Homes Program (HHP)

The HHP provides extra support for free. When you join the HHP a care coordinator is assigned to you to make sure that you get the healthcare and community services you need.

We can help with:
– Finding doctors and setting-up appointments
– Coordinating the care you get from different providers
– Helping you understand your prescription drugs
– Helping with food services
– Helping with transportation to and from medical appointments
– We can also help with housing and social services

How do I join the HHP?

Medi-Cal plans will contact members who qualify for the program. You can also ask your doctor or clinic about the program. You stay enrolled in your Medi-Cal Managed Care Plan and continue to see the same doctors, but you now have an added layer of support.

I’m a Michigan native who moved to California just a few years ago. I’ve been working for Titanium since 2019 as a Patient Care Coordinator – a job that I absolutely love!

I’ve had a very successful career as both a Psychotherapist and a Care Coordinator. I specialize in catastrophic injuries, psychological issues, homelessness, substance abuse issues and vocational rehabilitation. The majority of my work has been with auto injury victims and workers compensation cases.

I’ve been blessed to have owned two case management companies in Michigan, where I supervised and trained employees, conducted client psychosocial assessments, developed treatment plans, implemented interventions appropriate for the complexities of the case, coordinated and attended psychological, medical, vocational, housing, financial and support service appointments.

Helping patients navigate the medical system, and see their successes, is the most rewarding part of my job.

Being a Titanium Care Coordinator keeps me busy! I feel that we’re the advocating voice that our patients really need.

As a care coordinator, I keep primary care providers and other entities connected and informed about a patient’s ongoing health. We’re not only here to help patients navigate their healthcare, but to also utilize their community resources. We can provide patients with access to food banks, housing programs, and mental health services. A lot of the patients I work with weren’t aware of all of the resources that are provided in their community. It’s really rewarding to know that our patients and their loved ones can benefit from all the resources we can offer.

Care coordination is so impactful because it can help patients get their needs met and removes barriers.

Working at Titanium as a care coordinator has been one of the most rewarding experiences.

It has given me the opportunity to meet people from all walks of life and allowed me to make a positive impact. Working as a care coordinator has also allowed me to give back to my community by supporting the people in it.

The typical day of a care coordinator can be very fast paced, but we always have the support of our staff and fellow care coordinators. If we ever need support or guidance, we can always turn to a team member for assistance. I feel that the role of a care coordinator is important because it helps people feel more connected to their medical providers and builds a bridge between the patient and their doctor. The most rewarding part of the job is that moment when you help someone find the
answers or support they were looking for. Our role as care coordinators has a major impact on a person’s life because we provide the support they need to navigate through their healthcare.

Helping patients successfully navigate the Medi-Cal system is the most rewarding part of my job.
